Break the Excuse Pattern Program COACHING AGREEMENT
This agreement outlines the confidential coaching agreement between $FIRSTNAME$ $LASTNAME$ (“Client”) and Renée Remy, dba Dovetail Education (“Coach”).
CONFIDENTIALITY
- Coaching is totally confidential. The Coach will not divulge any Client information and will keep the Client’s identity confidential except as required by law.
DELIVERY OF COACHING AND TRAINING
- The 8-week Break the Excuse Pattern Program is delivered by a phone application, except for the weekly live pod meetings.
- You will be assigned to a learning pod that will meet for 45 minutes every week at a scheduled time. Please notify the coach in advance if you are unable to attend a pod meeting.
- You have two optional 30 minute coaching sessions available to be scheduled at the coach's convenience, to enhance your learning in this program.
- Coaching and pod meetings will take place by telephone or videoconference.
- The Client bears responsibility and cost for making the connection to the Coach at the agreed upon time and telephone number or conference system.
- Should the Client need to reschedule the coaching session, the Client will notify the Coach at least 24 hours in advance of the appointed time. The Coach reserves the right to not reschedule the missed session if notice is received less than 24 hours in advance of the appointed time. Sessions connected after the appointed time may be abbreviated at the Coach’s discretion.
- Should the Coach need to reschedule a session, the Client will be given at least 24 hours notice in advance of the appointed time. Otherwise the Client is entitled to an additional coaching session at no charge.
- Intellectual Property Rights – You may use the processes, techniques, presentations, methodologies, precedents and materials used or developed by Coach in the Coaching and Mentoring activities (“Materials”) only for your own personal use in connection with the Coaching and Mentoring activities. All intellectual property rights in the Materials are the property of the Coach. You shall indemnify the Coach for any loss or damage caused or sustained by it in the event of your breach of this paragraph.
FEES & PAYMENT & CLIENT AGREEMENTS
10. The Client understands that this program is being offered at the discounted rate of $497. The regular price for this program is $995.
11. The Coach will invoice the Client online, and Client shall pay by credit card or ACH transfer upon receipt.
12. It is the Client’s responsibility to pay for coaching in full and to independently seek any third-party reimbursements that may apply.
13. The Client agrees to allow coach to provide client's use of name and contact information to be provided to the Positive Intelligence Program certifying board, as needed.
WARRANTIES
14. The Break the Excuse Pattern Program application is licensed for use to Dovetail Education. Dovetail Education maintains no liability for Client's use of this application.
15. The Coach makes no guarantees or warranties, either expressed or implied, regarding the results to be achieved or the consequences of any actions taken or not taken by the Client.
16. Any decisions and actions by the Client are the sole and complete responsibility of the Client, and the Coach shall have no liability therein.
CANCELLATION OR CONTINUANCE
17. Should the Client decide that the program is not resulting in a benefit for the Client within the first 14 days of the start of the course, Dovetail Education will refund the fee paid for the course in full, upon request.
This agreement constitutes the full and complete understanding of both parties regarding the coaching relationship.
